
Illinois State Police (ISP) Division of Criminal Investigation Zone 1 Violent Crimes Unit special agents arrested 39-year-old Michael Vazquez of Oak Forest, IL for Aggravated Driving Under the Influence Involving Death (Class 2 Felony) stemming from a fatal crash in October 2024 at the Interstate 94 Illinois Tollway M4 maintenance building near Grande Avenue in Gurnee.
ISP patrol officers and Gurnee firefighters and paramedics responded about 7:32 a.m. Friday, October 18, 2024 to a report of a man run over by a skid steer at the Illinois Tollway M4 Building located near the northbound side of the Interstate 94 and Grand Avenue interchange in Gurnee. A man was found unconscious at the scene, and was transported to Level 1 Trauma Center Advocate Condell Medical Center in Libertyville, where he was pronounced.
The Illinois State Police investigated the incident that occurred on the Illinois Tollway property, which involved private contractor employees (not Tollway employees). The private contractors were laying asphalt. One of the private contractor employees, Michael Vazquez, was driving a skid steer when he backed up and ran over another contracted employee, identified as 39-year old Gregory Bolls of Chicago. Bolls was transported to an area hospital and later died of his injuries, according to the Illinois State Police.
According to the Illinois State Police, after a thorough investigation, it was determined Michael Vazquez had cocaine in his system at the time of the fatal skid steer vs pedestrian crash. The Lake County State’s Attorney’s Office on February 25, 2025, approved an Aggravated DUI charge and a warrant was issued for Michael Vazquez. On February 27, 2025, Vazquez surrendered himself to the Lake County Jail.
