Village of Arlington Heights Immunization Clinic Re-Opens August 12, 2013

The Village of Arlington Heights announced today that the Community Partnership Immunization Clinic will re-open Monday, August 12, 2013. The Clinic will be held from 5-7 p.m. in Health Services, which is located on the first floor of Village Hall, 33 S. Arlington Heights Road.

Immunization records and reservations are required one week prior to each clinic to ensure vaccine availability. These records can be mailed or dropped off at the Village of Arlington Heights, 33 S. Arlington Heights Road, Arlington Heights, Illinois 60005, or faxed to 847-368-5980.

Clinics will also be held on the following Mondays in 2013 – September 16, October 21, November 18, and December 16. The cost of vaccine for children is $10 per shot. Some adult vaccine is also available and also requires reservations and prior records. Please call for cost information. If you have any questions, please call 847-368-5760.
