One Killed, Five Ejected in Crash at Route 45 and Rollins Rd Near Gurnee and Third Lake, Warren Township

Lake County Sheriff’s deputies and Gurnee firefighters and paramedics responded about 6:40 p.m. Sunday, October 9, 2022 to a report of a crash with injuries and ejections at Route 45 and Rollins Road, Unincorporated Gurnee, for a traffic crash with injuries. Extra ambulances were dispatched based on reports of multiple people injured, and Newport firefighters were assigned to stand by in a Gurnee fire station. Lake County Sheriff’s deputies arrived and found five crash victims ejected from one vehicle.

Extrication was required for at least one crash victim.

Due to the number of victims injured and the extrication operation, firefighters and paramedics from Grayslake, Lake Villa, Libertyville, Round Lake and Zion were also assigned either to the scene or to stand by at Gurnee fire stations. A total of eight ambulances responded to the scene.




The crash involved three vehicles with 9 crash victims — eight were treated and transported, one of the transported victims died at the hospital, and one was treated at the scene and released. Five Mundelein residents traveling in a GMC Envoy were ejected upon impact. The Mundelein crash victims included a 2-year-old boy, a 4-year-old boy, a 24-year-old woman, a 31-year-old woman, and a 63-year-old woman. Those five patients were transported to Level 1 Trauma Center Advocate Condell Medical Center in Libertyville with critical to serious injuries. All of the ejected crash victims were stabilized and are expected to survive, according to the Lake County Sheriff’s Office. The driver of the GMC was also transported to Advocate Condell Medical Center with serious injuries. A 62-year-old front seat passenger on the impact side of the GMC Envoy was transported to Advocate Condell Medical Center, but pronounced deceased shortly after arriving at the hospital emergency department.




The driver of a Ford pickup truck with severe front-end damage was treated by paramedics at the scene. Two passengers in the Ford pickup truck — a 19-year-old man of Round Lake Beach, and a 19-year-old man of Round Lake Heights — were transported to Lake Forest Hospital with serious, non-life-threatening injuries.

Preliminary investigation shows the driver of the GMC Envoy — a 34-year-old man of Mundelein — was traveling westbound on Rollins Road. He was attempting to make a left turn onto southbound Route 45 while turning left on a yellow light when the driver’s GMC Envoy was struck by an eastbound Ford pickup truck, driven by a 22-year-old man of Round lake. The crash occurred in the intersection. A westbound Toyota Venza was struck by debris immediately following the crash. None of the occupants of the Toyota Venza were injured.




Apparently, the children traveling in the GMC Envoy were sitting on the laps of two of the backseat passengers. All of those ejected from the GMC Envoy were unrestrained, according to the Lake County Sheriff’s Office.

The crash remains under investigation by the Lake County Sheriff’s Office Technical Crash Investigations Unit. The intersection is a slightly oblique intersection with two lanes and a left turn lane in each of the Rollins Road directions (five lanes total in each direction). The left turn lanes on Rollins Road also have a painted area separating the two straight-through lanes from the left turn lanes. The Speed Limit on Rollins Road is 40 MPH, according to Google Maps Street View.

North of the Intersection
Southbound Route 45 has three lanes approaching the intersection — a left turn lane, a straight through lane, and a right turn only lane. Route 45 has one lane northbound departing the intersection.

South of the Intersection
Northbound Route 45 also has three lanes approaching the intersection — a left turn lane, a straight through lane, and a right turn only lane. Route 45 has one lane southbound departing the intersection.

The Speed Limit on Route 45 north and south of the Rollins Road intersection is 45 MPH according to Google Maps Street View.

Route 45 runs on a slight ridge near the intersection, which forms a slight crest that can obscure oncoming traffic from either direction on Rollins Road.
