Shruti Phadnis, 26, Dies 5 Days After Head-on Crash Near Cubby Bear, Lincolnshire

Shruti Phadnis, 26, of Buffalo Grove has died at Level I Trauma Center Advocate Condell Medical Center in Libertyville, nearly five days after she was transported there from the scene of a head-on crash Friday night, January 13, 2012 about 8:50 p.m. Phadnis was a passenger in the front seat of a Toyota sedan. She married Devang Vora the day before on January 12, 2012.

Buffalo Grove police received a report about 8:42 p.m. Friday January 13, 2012 that a silver Honda CRV took out a street sign and was crashed into a snow bank on Half Day Road near Buffalo Grove Road. Police arrived and discovered a large street sign in the road, but the vehicle was no longer on the scene. Meanwhile, Buffalo Grove and Lincolnshire police received multiple calls about a reckless driver traveling the wrong way in lanes of Milwaukee Avenue — heading southbound. A Lincolnshire police officer witnessed the vehicle pass southbound at a high rate of speed. About 8:51 p.m. Friday Buffalo Grove police received a report that their vehicle that crashed on Half Day Road was likely involved in a head-on crash in front of Cubby Bear on Milwaukee Avenue.


The head-on crash involved three vehicles in front of Cubby Bear North, 21661 North Milwaukee Avenue. A silver Honda CRV, a dark green Toyota sedan, and a Jeep SUV were found crashed in the middle of Milwaukee Avenue.

The driver of the Honda CRV is still hospitalized. The driver of the Jeep SUV and the Toyota were treated and released from the hospital.

Lincolnshire-Riverwoods FPD firefighter/paramedics responded and transported crash victims to area hospitals.

Police were investigating the crash scene with Milwaukee Avenue closed past midnight. Lake County Major Crash Assistance Team was investigated the Milwaukee Avenue scene and the Half Day Road scene.
