Crystal Lake and Nunda Township firefighters 3:29 PM Wednesday to a report of a house fire at 3217 Carrington Drive Crystal Lake, IL. Firefighters arrived to find one home fully-involved with flames and embers landing on the roofs of at least three homes across the street to the north. A fourth home, on East Terra Cotta Avenue, was not visible from the main fire scene, but also received minor damage from embers landing on the home.

With the first house at a total loss, firefighters concentrated efforts on the homes to the north of the initial house, which was totally destroyed with only a chimney and one partial side of a brick wall still standing. A gas line continued to burn with an associated hissing or rushing sound from the initial home on the northeast side of the home.

Neighbors were quick to assist by attempting to soak the roofs of the affected homes and help evacuate their neighbors. One man was burned slightly when he was hit by an ember. Neighbors also helped the resident who lives at 3217 East Terra Cotta. A Woodstock fire engine and brush trucks from Richmond and Wonder Lake were positioned to protect the house on East Terra Cotta Avenue.

A Box Alarm to the second level was immediately called, but then was upgraded directly to a fifth alarm. A Box Alarm to the third level for tenders (tanker trucks) was also requested.

Tanker trucks from several fire departments shuttle water to portable tanks setup in the streets.

3222 Carrington was left uninhabitable after heavy fire damaged the cedar shake roof, extended into the attic, and resulted in open holes in the roof. The interior of the house suffered smoke and water damage.

3216 Carrington received fire damage to the cedar shake roof and some interior smoke damage.

3306 Carrington received fire damage to their cedar shake roofs and some interior smoke damage.

3217 E. Terra Cotta Avenue, the next roadway north of the neighborhood received minor damage to the rear deck and siding. Landscaping damage also occurred with burning plants and other outdoor structures between the backyards of Terra Cotta Avenue and Carrington Drive.

Landscaping spot fires also occurred between the backyards of the initial fire residence at 3217 Carrington Avenue and the house directly south of the fire.

No initial heat damage was discovered at the house directly to the south of the initial house burned to the ground.

Winds at 3:52 p.m. at DuPage Airport in West Chicago were reported from the South at 13 MPH. At 4:52 p.m. winds were reported from the South at 10 MPH with gusts to 18 MPH. Air temperature during initial firefighting operations was 83-84°F.

Three firefighters were transported to Centegra Hospital in McHenry with heat-related illnesses.

Firefighters from Algonquin-Lake in the Hills, Barrington, Cary, East Dundee, Fox River Grove, Hebron-Alden-Greenwood, Huntley, Lake Zurich, Long Grove, Marengo, McHenry Township, Palatine Rural, Richmond, Rutland-Dundee, West Dundee, Wonder Lake, Woodstock, Union and others assisted Crystal Lake and Nunda Township firefighters.

Valley View Road between East Terra Cotta Avenue and East Crystal Lake Avenue was closed for staging of firefighting equipment.

VIDEO: Prospect Heights Crash Truck 39 and ambulances respond to a crash involving a truck and two cars, which caused at least one of the truck’s gas tanks to cause a fire (Video taken from the Frontage Road on the west side of Edens Expressway/Skokie Highway/I-94 — CREDIT: YouTube.com/brian60093).

Northfield firefighter/paramedics responded about 9:09 p.m. to a truck-car crash with a truck on fire on top of a car in the northbound lanes of the Edens expressway. Firefighters reported seeing flames while responding to the fire. Immediately on arrival, firefighters called for the Northfield ladder truck and a Prospect Heights crash truck stationed at Chicago Executive Airport.

The crash was located in the northbound lanes of the Edens Expressway, just north of the Willow Hill Executive Center, which is located on the opposite side of the expressway on the Frontage Road.

Apparently a truck traveling northbound changed lanes from a right lane toward the center lane and struck a car. The truck then veered right and struck a second car, driving over it. The truck then came back left and hit the median wall of the northbound expressway lanes of the Edens Expressway (Interstate 94).

An EMS Box was requested bringing a total of two ambulances from Northfield, an ambulance from Glencoe, an ambulance from Nortbrook, and an ambulance from Morton Grove. At least one victim was transported Advanced Life Support to Evanston Hospital with third degree burns. Three other people were transported with minor injuries to unspecified local hospitals.

Both the northbound lanes and the southbound lanes of the Edens Expressway were closed during crash/rescue operations.

VIDEO: Multi-vehicle crash on I-294 over Central Road, crashes on Palatine Road, crashes on Route 53, and an extrication crash on I-290 over Addison Road.

Graupel or snow pellets fell about 10:00 p.m. Friday causing very slick roads. The pellets are extremely slippery when they are fresh on the streets. They often melt and then form black ice, especially on overpasses and bridges on interstates. That’s what happened overnight Friday. Traffic ground the pellets into dangerous ice on the roads. On most of the interstate and expressway, the bridge surfaces were frozen while the roadways were wet.

Palatine Road was immediately closed when crashes started occurring about 1:10 a.m. Wheeling police and Prospect Heights police were on each end of the Elmhurst Road (Route 83) bridge at Wheeling Road and Schoenbeck Road, respectively. Minutes later Glenview Fire Department requested an EMS Box at Interstate 294 in both the northbound and southbound lanes. Firefighter/paramedics from Morton Grove, Mount Prospect, Niles, North Maine, and Park Ridge responded to I-294. Prospect Heights firefighter/paramedics were kept busy on Palatine Road over Milwaukee Avenue.

Some drivers were whizzing by the crash scene at over 55 mph, apparently believing they were somehow immune to the fate of the other drivers that had crashed or were facing the wrong direction in the expressway lanes.

A crash with the center median wall also occurred on Route 53 over Palatine Road and on Route 53 over Northwest Highway — both crashes were in the southbound lanes. Minor injuries or no injuries were reported. At least two crashes also occurred on southbound Route over Interstate 90. The crash on Route 53 over Northwest Highway involved a dark vehicle unattended in a dark area with the rear end of the vehicle sticking out into the inside lane — very dangerous.

An extrication crash also occurred on westbound I-290 over Addison Road, just west of the Route 83 interchange. Both a van and a sedan were faced in the wrong direction at the median wall. The sedan already had it’s roof cut off, and Illinois State Police investigation was underway about 5:30 a.m.

Inbound and outbound train movement was halted near Des Plaines due to the pedestrian incident involving train #618 about 7:30 a.m. The duration of this delay is unknown but extensive delays are expected. The westbound train is stopped just before Pearson Street at the Union Pacific crossing grade.

Niles police and firefighter/paramedics 2:00 AM Saturday responded to report of a rollover crash into a house Seward St and Oakton Ct Niles, IL. Police and firefighter/paramedics arrived to find a car that hit a tree and was resting against a house on Seward St and Oakton Ct. The vehicle apparently left the roadway from Caldwell Avenue, just north of Oakton Street.

Timothy Thomas, age 36, of North Marmora Avenue was found dead at the scene.

Police are investigating the crash. From the appearance of the area cordoned off by yellow police tape, the vehicle was northbound on Caldwell Avenue from Oakton Street when it left the roadway, hit a medium-size tree and came to rest against a house on the north side of Seward Street at the corner of Seward Street and Oakton Court.

A NORTAF vehicle was on the scene for investigation of the fatal crash.

A Lake Zurich public works employee initially tried to reach the stranded teenage boy, but he returned to shore because the water was too cold.

The teens were walking on the ice with a 9-year-old boy and a 7-year-old girl when they fell in the icy water. The younger and lighter children were able to gain footing on the ice, and walked toward the shore on their own. The teens were stranded on an ice shelf about 100 yards from shore. Both teens were rescued by firefighters wearing cold-water rescue suits and carrying personal flotation devices just after 3:00 p.m. The teens were transported to Advocate Good Shepherd Hospital with signs of minor hypothermia.

The two younger children were released to their parents after they were checked by paramedics.

Firefighter/paramedics from Arlington Heights, Long Grove and Palatine Rural Fire Protection District were called in to help about 2:50 p.m. — either at the scene or at fire stations for standby coverage of Lake Zurich.

Ice is much weaker in late winter and early spring, and especially dangerous in open water. Sometimes people can manage to crawl or slither on ice so they don’t fall through, but even an elbow can punch through the water an immerse a victim’s face or a rescuers face into the ice cold water.

The images apparently show a man wearing a dark hoodie, dark head and facial cover (possibly a dark Balaclava), dark over-the-wrist long gloves with a white logo, one white shoe and one dark-colored shoe. Police said he appears to be a white male about 5-foot-9 to 6-foot tall, holding a large knife. The offender was seen removing currency from the cash register. The gloves appear to be black work gloves, or sports, ski or snow mobile gloves.

The offender appears to be armed with a combat or fighting knife — possibly a black KA-BAR Tanto knife or similar knife. The KA-BAR Tanto knife is described by the manufacturer as a “thick pointed blade that’s good for penetration.” The knife blade of the Tanto knife is 8 inches, and the overall knife length is 12-7/8 inches.

Streamwood police urge anyone with information to call their investigations division at 630-736-3700 or leave an anonymous tip at 630-736-3719.

Metra train that was involved in accident that struck an 11-year-old boy as it was stopped near the Barrington Metra station (submitted by a commuter at the scene Friday night).

Barrington police and firefighter/paramedics responded about 6:05 PM Friday to a report of person hit by a train at the Mertra Station Barrington (one block north). Several people were helping an 11-year-old boy who suffered serious-to-critical injuries when firefighter/paramedics arrived. The boy, who suffered a severed foot, was transported to Level I Trauma Center Advocate Lutheran General Hospital in Park Ridge.

EXPLOSION CAUGHT ON VIDEO (6:52): Explosion with debris flying and heavy smoke from a commercial building where paint and paint striping systems are manufactured at 640 Industrial Drive in Cary, Illinois (VIDEO CREDIT: 3GSTORE.COM).

Three people were seriously injured at a paint factory explosion on Industrial Drive in Cary Tuesday after, but fortunately are expected to survive the injuries. All three victims, Jose Martinez, 28; Jorge Anastio, 28; and Robert Hance were transported to Advocate Good Shepherd Hospital near Lake Barrington. Two of the victims, Jose Martinez and Robert Hance, were transferred by medical transport helicopters to the hospital burn unit at Loyola University Medical Center in Maywood. Both of the victims were listed in serious condition at Loyola University Medical Center. Jorge Anastio was transferred to Saint Anthony Medical Center in Rockford. He is listed in fair condition.

Witnesses on the block reported seeing one man rolling on the ground at the front of the building immediately after the explosion. Several smaller pops and explosions occurred, and one larger explosion occurred about 7 minutes into the incident. Heavy black smoke rolled out of the commercial building. A brick wall at the front of the building was also blown out and destroyed.

Components used in the mixture of colorful paints made at the factory are highly flammable, and can be ignited from exposure to a static spark or even a hot light bulb.

Cary firefighers responded about 1:10 p.m. Tuesday, March 6, 2013 to an explosion and fire at 640 Industrial Drive. Witnesses on the block reported seeing one man rolling on the ground at the front of the building immediately after the explosion. Witnesses on the block also said it sounded like a plane crash. Several smaller pops and explosions occurred, and one larger explosion occurred about 7 minutes into the incident. Heavy black smoke rolled out of the commercial building. A brick wall at the front of the building was also blown out and destroyed.

Three people were transported to hospital(s) in the area. At least one person was transported to Advocate Good Shepherd Hospital. The victims are reported to have non-life-threatening injuries.

Two fire department aerial towers were seen streaming water into the building near two large white tanks from a television news helicopter.

About 90 minutes after the explosions with smoke still rising from the building, a HazMat extra alarm for HazMat technicians was requested. A special request for a foam truck from Crystal Lake Fire/Rescue and a Mobile Ventilation Unit from MABAS Division 1 were also called to the scene.

Firefighters from Barrington, Cary, Crystal Lake, Fox River Grove, McHenry Township, and Nunda Township responded to the second alarm. Firefighters from Elgin, Long Grove, and Quad 2 Fire Departments Foam Response Unit also responded. HazMat techs from as far as Buffalo Grove responded to the HazMat Box Alarm.

Several people working at 3GSTORE.COM across the street immediately grabbed cameras, and wondered whether they should evacuate — even before firefighters asked them to evacuate. 3GSTORE sells mobile solutions, including mobile networking devices, Wi-Fi signal boosters and antennas, and mobile hotspots. Some of the devices are used in mobile situations, such as during fireground operations.

Four ambulances responded to the crash scene with mutual aid from Carpentersville and Palatine Rural firefighter/paramedics.

Extra police officers from the area were called in to block roads in the area. Westbound Dundee Road traffic is being diverted to northbound Route 59 (Hawthorne Rd). Northbound traffic is blocked at the Hawthorne Road-Dundee Road-New Sutton Road-Algonquin Road intersection. No right turns allowed from westbound Algonquin Road to northbound Hawthorne Road (Route 59) and no left turns allowed to go northbound on Hawthorne Road (Route 59).

Fire command ordered a Life Safety Box at the address and requested medical helicopter transport for at least two victims. Two medical helicopters were disptached — one from University of Chicago, and one from LifeStar, which is based at Loyola University Medical Center or the Joliet area — according to their website.

The danger was discovered when a boyfriend of a 22-year-old daughter of the family arrived at his girlfriend’s house about 5:55 a.m. They had spoken earlier, and the girl told him that she was feeling sick and was vomiting. When the boyfriend arrived, he saw that his girlfriend’s father was passed out on the floor. He called 9-1-1. Firefighters forcibly entered the home and found three more people and the family dog — all unconscious. All were evacuated, and all were revived. Two victims were transported to St. Catherine’s Hospital in Kenosha, Wisconsin; and two victims were transported to Aurora Medical Center in Kenosha County.

The family’s 13-year-old daughter was transferred by medical helicopter from St. Catherine’s to Loyola University Medical Center, Maywood, for Hyperbaric Oxygen Therapy to force the carbon monoxide out of her blood and to increase the amount of oxygen that can be dissolved in her blood plasma.

The dog was transported to an animal hospital.

No smoke detectors were discovered inside the home, but firefighter equipment measured a high level of carbon monoxide at about 600 part per million. Carbon monoxide at 400 ppm is considered life-threatening after three hours of exposure. Levels of 1500 ppm are considered immediately dangerous to health, with death possible within one hour of exposure.

Home-installed carbon monoxide (CO) detectors will activate in a few minutes at 400 ppm. At lower concentrations, alarms might not activate for ten, twenty or thirty minutes. Some detectors activate when 40-70 ppm or more are detected. Some alarms are set to activate after 30 minutes at 100 ppm, and after 10 minutes at 200 ppm.

Carbon monoxide (CO) is an odorless, colorless gas, which can cause sudden illness and death. CO building up in a house and poisoning can result from a malfunctioning furnace and ventilation, a fire, or a vehicle running inside a garage; or CO buildup inside a running vehicle can occur while the exhaust is blocked.

A faulty boiler is suspected to be the source of the carbon monoxide at the Antioch home, but the cause is still under investigation.

Aurora Medical Center is known to have a Hyperbaric Chamber for the treatment of acute carbon monoxide poisoning. There is no confirmation whether any of the other victims received any hyperbaric treatment.

Hyperbaric Oxygen Therapy (HBOT) is known as an advanced therapy for acute carbon monoxide poisoning. Medical hyperbaric oxygen therapy delivers a carefully managed dosage of pressure and pure oxygen over a prescribed period of time. Treatment periods are called “dives” as HBOT is also used to treat decompression sickness or “the bends” which can occur when divers rise to the water surface too quickly.

Hyperbaric oxygen therapy is delivered in a chamber pressurized at 1.4 to 3.5 times the normal sea-level atmospheric pressure of 14.696 pounds per square inch.

CO locks out oxygen from its normal function of transport via hemoglobin.
Under higher pressures, (hyperbaric), oxygen dissolves in the blood plasma, and has a better chance of getting back to normal binding with hemoglobin for transport to to all body tissues that is necessary for life. Breathing pure oxygen in a hyperbaric chamber increases the oxygen-carrying capacity of blood, and is used to counter the harmful binding effects of CO on hemoglobin.

Several hospitals in the area have hyperbaric chambers, including Level I Trauma Center Advocate Lutheran General Hospital and Froedtert Memorial Lutheran Hospital.

Washington.

The first unit arrived on the scene at 8:08 AM and was directed to the top level of the construction project where they found the injured worker. Due to the limited access to the upper level of the building’s addition, a Specialized Rescue Response was requested by the first-in company officer. The Specialized Rescue Response brought another 10 personnel and 5 pieces of fire apparatus to the scene. The fire companies treated the patient, secured him in a patient packaging device and used the on-site crane to lower him to the ground for transport to the Emergency Department.

The Naperville Fire Department was assisted at the scene by the Naperville Police Department.

Rescuers received a report that a vehicle rolled over “two or three times” during the crash. A witness also reported CPR was underway on the roadway. The crash occurred exactly at the 47 Mile Marker sign.

There was also report of a second victim — a female — another one-half mile south of the first crash scene, but this was apparently misinformation. One person is confirmed dead after being ejected from a vehicle. The victim, identified as Danielle M. Pisterzi, 21-years-old of Chicago, was transported to Level I Trauma Center Advocate Lutheran General Hospital. She was pronounced dead at 11:52 p.m. Saturday.

Initially police and firefighters received a report that a child was in the roadway. There appeared to be some confusion in dark conditions with several uninjured people standing near the crash scene. Only one victim was transported to a hospital from the scene.

Southbound I-294 was entirely shut down at Willow Road. All traffic was required to exit at Willow Road. Vehicles that were blocked by the crash, and were already south of Willow Road, were diverted back north in the southbound lanes to leave an exit at Willow Road.

An extended accident investigation was underway overnight. Southbound I-294 re-opened at about 4:00 a.m.

Glenview police and firefighter/paramedics responded about 6:00 a.m. to a crash near Spruce Street and Glenview Road involving a Glenview Park District snow plow and a Pace bus. The Pace bus went through a hedge, and the snow plow broke off the truck and hit an Audi.

Minor injuries were reported by the Pace bus driver, the only passenger on board, and the driver of the Audi.

Northbrook firefighter/paramedics and Illinois State police responded about 6:47 a.m. Sunday to a report of a crash involving a taxi and a tow truck at the 52 and 3/4 mile marker on the southbound Tri-State. The crash was located at the split in the inside lane of the southbound split to I-294, just north of Lake Cook Road. Bystanders told firefighters one person was ejected or thrown from a vehicle. Firefighters discovered one person on the ground. The crash possibly involved a tow truck, a taxi cab, and one other vehicle.

Minutes after arrival firefighters reported they had and extrication operation underway and requested a second ambulance and another fire engine for additional personnel for the extrication operation. Five ambulances were eventually called to the scene with two staging off of the Tollway for a part of the rescue operation.

One person was pronounced dead at the scene. Three crash victims were transported to Level I Trauma Center Advocate Lutheran General Hospital. A fourth victim was transported to Highland Park Hospital. The four people transported had non-life-threatening injuries.

Lincolnshire-Riverwoods firefighters/paramedics assisted Northbrook firefighters. Firefighters remained at the scene past 9:00 a.m. assisting Illinois State Police with their investigation.

Southbound lanes to I-294 were still blocked as of 9:46 a.m. Sunday.

Chicago police, firefighters and paramedics responded about 1:25 a.m. Tuesday to a car vs. median crash in the 2000 block of West Madison Street between Damen Avenue and Hoyne Avenue. Shawna Daro was driving a 2002 Scion when the vehicle hit a concrete median that holds flowers — located just east of Hoyne Avenue, almost two blocks west of the United Center in Chicago. She was trapped in the vehicle, but extricated by rescuers about 1:35 a.m.

Shawna Daro, 30, of West Miner Street in Arlington Heights, was transported with Advanced Life Support to Level I Trauma Center John H. Stroger Jr., Hospital of Cook County, where she was pronounced dead about thirty minutes after arrival.

Visitation Monday, February 4, 2013, from 3:00-9:00pm at Smith-Corcoran Funeral Home, 185 E. Northwest Hwy., Palatine. Family and friends will meet Tuesday at Holy Name Cathedral, 735 N. State St., Chicago, for visitation from 9:30am until time of mass at 10:30am. Interment will be private.

Firefighter/paramedics entered a home with a kitchen fire that was unoccupied, except for a dog and another pet inside the home. Firefighters rescued the two pets and discovered the stove and cabinets on fire.

During investigation, it was discovered that one of the dogs had a history of jumping up on the stove and turning on the stove top burners. Monday morning a pizza box was on the stove, and the dog apparently jumped up to check out the pizza box and hit the stove top knob, which ignited the pizza box.

South Elgin and Countryside Fire Protection District firefighters called for a MABAS Box Alarm, which was struckout by 9:30 a.m.
